Personal Account Checklist

As a resident of Tuscarawas County, you are eligible to join DoverPhila. Also, anyone who works in, worships in, or attends school in Tuscarawas County is eligible for membership. Once you become a member, we gladly extend our credit union membership perks to any member of your immediate family.

Opening any personal account at DoverPhila will require individuals to present the following:

Unexpired, Government-Issued Photo ID

  • Out of state ID requires proof of local address*
  • D. issued within last 30 days requires proof of address*
  • If address on ID is not correct, we need proof of correct address*

Identification Number

  • For U.S. Person-TIN, SSN, ITIN, EIN (SS Card, W-2, IRS official document)
  • For non-U.S. person: Please speak to a Front Desk representative

*Acceptable proof of address: bank statement, check stub from payroll or Government issued check, utility bill, lease agreement, Ohio concealed weapons permit, Income tax return filing, proof of home ownership.


In the case of a minor, the elderly or handicapped, Amish, or another individual who cannot produce a photo ID, we will accept 2 of the following:

  • Last three months of utility bill with correct name and address
  • Other photo I.D.
  • Birth certificate
  • Social Security card
  • Confirmation of identity from another individual on their behalf
  • Other manager approval methods on a case-by-case basis

Business Account Checklist

Opening any business account at DoverPhila will require specific documents to be presented consistent with entity’s designation. Please review carefully to know what documents are required for your designation.

An appointment may be needed to open an account in person. Call 330-364-8874 to make arrangements. 

Corporations, LLCs, Partnership, & Non-Profits

  • State of Ohio Registration
  • Proof of TIN-from IRS
  • Driver’s License/ photo ID of all owners/agents
  • Proof of SSN of all owners/agents
  • Partnership agreement (partnership only)

Credit Union will provide paperwork for business resolution as well as Beneficial Ownership Certification. 

Sole Proprietor/DBA

  • Proof of TIN/SSN
  • Letter of Authority (we can provide)
  • Driver’s License/Photo ID

Association, Club, Religious, & Charitable Organizations

  • Letter of Authority (we can provide)
  • Proof of TIN
  • 501c3 if charitable organization is non-profit
  • Driver’s License/Photo ID of agents

Trust

  • Proof of TIN or SSN
  • Trust document
  • Proof of SSN for all new trustees
  • Valid driver’s license or photo ID for all new trustees

Estate

  • Proof of TIN
  • Court appointed executor/administrator documents
  • Death certificate
  • Valid driver’s license or photo ID for executor/administrator
  • Proof of SSN for executor/administrator

All locations will be closed on Thursday, November 28, and will close at 2:00pm on Friday, November 29. Our North Dover, New Phila, and Newcomerstown branches are drive-thru only on this day. Normal business hours will resume on Saturday, November 30.
